Anatomy Identification (ID) Module   [Mpeg] [MOV]

The trainee will be given verbal prompts to identify the key anatomical landmarks associated with performing a cystoscopy in preparation for TURP.  The trainee then identifies the structure by touching the loop to the structure.

 

Objectives:

1.      Teach anatomy of the lower urinary tract as seen endoscopically (basic cystoscopy)

2.      Orientation using an actual resectoscope in a 3-dimensional working space given 2-dimensional visual cues

3.      Identification of landmarks and hazards to avoid during TURP

 

Metrics:

·        Time to task

·        Number of correct and incorrect responses

 

Cutting Module [Mpeg] [MOV]

After being shown one of a series of different sized virtual 3-D objects, the trainee uses the footpedal cutting current to cut from within the object to an edge as close as possible in a given amount of time.

 

Objectives:

1.      Dealing with obstructing chips

2.      Cutting efficiency and accuracy

3.      Coordination of both hands and foot-pedal during real time simulation

4.      Assessment of visuo-spatial skills

 

Metrics:

·        Time to task

·        Number of chips

·        Grams resected

·        Area of perforation through object

·        Times cut pedal depressed

·        Grams/Cut

·        Number of perforations outside the object

·        Amount of time spent with a sticky chip

·        Amount of charring of tissue

 

 

Coagulation Module  [Mpeg] [MOV]

Working in a cylinder, bleeding vessels appear along the surface and the trainee coagulates them using the coagulation foot pedal.  As time progresses, the bleeding vessels appear faster and faster requiring the trainee to use the irrigation in order to prevent red-out.  The task ends when red-out occurs.

 

Objectives:

1.      Dealing with issues of hemostasis

2.      Management of fluid irrigant using inflow and outflow stopcocks

3.      Coordination of both hands and foot-pedal during real time simulation

 

Metrics:

·        Number of bleeders coagulated

·        Average amount of time taken to coagulate bleeders

·        Amount of irrigant used

·        Total blood loss

 

Advanced Resection Module  [Mpeg] [MOV]

The trainee will be performing a complete anatomical operation. Will have to address issues related to camera and loop orientation, footpedal controlled cutting and coagulation and stopcock controlled fluid management. 

 

 

Objectives:

1.      Address issues related to camera and loop orientation

2.      Footpedal controlled cutting and coagulation

3.      Refining ability to minimize blood loss and efficiently use fluid irrigant

4.      Studying and acquiring more efficient patterns and styles of resection (Pearls)

·        Cut and Coag count
